Physiotherapist – Band 6

Brief Job Overview

Tripod Partners are recruiting an experienced Band 6 Physiotherapist for a locum opportunity within East Riding of Yorkshire NHS. Based with City Health Care Partnership CIC, this community rehabilitation role supports adults across Hull and the East Riding, delivering high-quality physiotherapy interventions within patients’ own homes and community settings.

This is an excellent opportunity for a community Physiotherapist who enjoys autonomous working while collaborating with multidisciplinary teams to help people maximise their independence, improve mobility, and achieve their rehabilitation goals. You will manage a varied caseload, undertaking home visits and providing evidence-based rehabilitation across the local community.

Key Responsibilities

• Provide specialist physiotherapy assessments and treatment for adults within community settings.
• Manage an independent community rehabilitation caseload across Hull and the East Riding.
• Undertake home visits to assess mobility, functional ability, and rehabilitation needs.
• Develop and deliver personalised, evidence-based rehabilitation programmes.
• Promote independence and support individuals to achieve their functional goals.
• Work collaboratively with multidisciplinary teams, GPs, community services, and carers.
• Monitor patient progress and adapt treatment plans in response to changing clinical needs.
• Maintain accurate and timely clinical documentation in accordance with HCPC and NHS standards.
• Contribute to safe discharge planning and ongoing community rehabilitation pathways.
• Deliver high-quality, person-centred physiotherapy services within the community.

About You

You will be an experienced HCPC-registered Physiotherapist with a strong background in community rehabilitation and NHS practice. You will be confident working independently, managing your own caseload, and travelling throughout the community to deliver high-quality rehabilitation services.

You will possess excellent clinical reasoning, communication, and organisational skills, together with the flexibility to work across a range of community locations.

Requirements

• Degree or Diploma in Physiotherapy.
• Current HCPC registration.
• Relevant UK NHS Physiotherapy experience.
• Minimum of 2 years’ community rehabilitation experience.
• Experience managing an independent community caseload.
• Ability to undertake home visits across Hull and the East Riding.
• Full UK driving licence and access to a vehicle.
• NHS email account.
• Excellent assessment, rehabilitation, communication, and multidisciplinary working skills.
