Devon County Council is seeking a committed and qualified Adult Social Worker to join our Adult Care and Health team. This role involves working with adults who have care and support needs, including older people, those with physical disabilities, learning disabilities, or mental health conditions. You will support individuals to live independently and safely, while ensuring their rights and dignity are upheld under the Care Act 2014.
Key Responsibilities:

Undertake person-centred assessments and reviews in line with statutory duties under the Care Act 2014.

Develop and implement tailored care and support plans to promote independence, choice, and well-being.

Carry out safeguarding enquiries under Section 42 and work collaboratively with partner agencies to protect adults at risk.

Maintain accurate and timely records in accordance with Devon County Council policies and professional standards.
