Undertake statutory assessments, support planning, and safeguarding enquiries in accordance with the Care Act 2014, Mental Capacity Act 2005, and related legislation.

Manage a diverse caseload involving older adults, people with physical disabilities, learning disabilities, and mental health needs, ensuring personalised and proportionate interventions.

Work collaboratively with service users, families, carers, health professionals, and community partners to develop integrated, outcome-focused support plans.

Promote independence, choice, and wellbeing through a preventative approach that supports individuals to remain in their own homes and communities.
