Join Stockton-on-Tees Borough Council’s Adults’ Services as a Qualified Social Worker, supporting adults with a range of needs to live independently, safely, and with dignity.

Undertake person-centred, strengths-based assessments in line with the Care Act 2014, working with individuals and carers to co-produce tailored support plans.

Manage a diverse caseload, including older adults, people with disabilities, and those with mental health needs, ensuring timely interventions and appropriate support.

Promote safeguarding by identifying and responding to risks of abuse or neglect, participating in multi-agency safeguarding procedures when necessary.

Collaborate effectively with health professionals, voluntary services, and community networks to deliver coordinated, outcome-focused care and support.
