Join Durham County Council’s Adults’ Services as a Qualified Social Worker, supporting adults with care and support needs to live independently, safely, and with dignity.

Undertake strengths-based assessments in line with the Care Act 2014, co-producing personalised care plans that promote wellbeing and autonomy.

Manage a diverse caseload, including older adults, individuals with physical or learning disabilities, and adults with mental health needs, ensuring effective support and timely interventions.

Work in close collaboration with families, carers, and multi-agency partners to deliver coordinated, outcome-focused support across health and social care.

Respond to safeguarding concerns, contribute to multi-agency enquiries, and uphold the principles of the Mental Capacity Act 2005 in all practice.
