Job Title: Occupational Therapist or Physiotherapist – Band 6

Job details: Occupational Therapist or Physiotherapist required to start ASAP for Community Therapy Wards.
- Provide therapy input within community-based hospital wards
- Undertake mobility assessments and develop individualised exercise programmes
- Support rehabilitation and reablement of patients
- Contribute to effective discharge planning to support safe transitions from hospital
- Provide and prescribe appropriate equipment, including manual handling equipment
- Supervise and support junior members of staff
Skills & experience required:
- HCPC registered Occupational Therapist or Physiotherapist
- Experience working on hospital wards within a rehabilitation or reablement setting
- Strong skills in mobility assessment, exercise prescription and discharge planning
- Manual handling experience essential
- Knowledge of common conditions including arthritis, heart disease, diabetes, dementia, MSK conditions, COPD, orthopaedics and falls
- Car driver essential – role requires transporting items of equipment
