Stroke Occupational Therapist – Band 7

Tripod Partners are recruiting a Band 7 Stroke Occupational Therapist for a locum role within South West Wales NHS. This position sits within a specialist stroke service, supporting early intervention, rehabilitation, and complex discharge planning for patients following stroke.
The Occupational Therapist will provide specialist assessment and intervention for individuals experiencing cognitive, motor, and functional difficulties following stroke. The role focuses on delivering timely rehabilitation to support recovery, maximise independence, and enable safe discharge through a home-first approach.
Key Responsibilities
- Carry out specialist occupational therapy assessments for patients following stroke.
- Assess cognitive, motor, and functional difficulties impacting daily living activities.
- Deliver rehabilitation interventions to improve independence and quality of life.
- Support early intervention and complex discharge planning from acute services.
- Contribute to home-first and discharge-to-assess pathways.
- Work collaboratively with multidisciplinary teams to coordinate patient care.
- Support patients to retain or reduce reliance on care packages where appropriate.
- Provide advice and support to patients, families, and carers.
- Maintain accurate clinical documentation in line with NHS standards.
About You
You will be an experienced Occupational Therapist with specialist knowledge in
stroke rehabilitation and complex discharge planning. You will demonstrate strong
clinical reasoning skills and the ability to manage patients with complex neurological and functional needs.
You will be confident working within a multidisciplinary environment and delivering person-centred
interventions that support rehabilitation and independence.
Requirements
- Recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Experience within stroke rehabilitation services.
- Experience in complex discharge planning and rehabilitation.
- Ability to manage a specialist caseload independently.
- Strong communication, organisational, and clinical reasoning skills.
