Buckinghamshire Council is seeking a compassionate and experienced Social Worker to join our Adult Community Team. This role involves working with adults who have diverse needs, including older adults, those with disabilities, and individuals with mental health conditions. You will provide person-centred assessments and interventions, promote independence, and work collaboratively with partners to improve the quality of life for service users within the community.
Key Responsibilities:

Conduct comprehensive assessments under the Care Act 2014 to identify support needs and develop tailored care plans.

Respond to safeguarding concerns in line with Section 42 of the Care Act, ensuring adults at risk are protected.

Collaborate closely with health services, housing, voluntary organisations, and families to coordinate effective care and support.

Apply the Mental Capacity Act 2005 to assess decision-making capacity and support best interest decisions.

Maintain accurate, timely records and reports that meet Buckinghamshire Council’s standards and statutory requirements.
