View Job-
Carry out strengths-based assessments under the Care Act 2014 to identify eligible needs and develop personalised support plans for adults and older people.

Manage a varied caseload, including safeguarding enquiries, hospital discharges, mental capacity assessments, and Deprivation of Liberty Safeguards (DoLS) work.

Work collaboratively with health professionals, housing, voluntary services, and other partners to deliver integrated, person-centred support.

Ensure timely and accurate recording of all casework on Mosaic (or relevant case management system) in line with Council policies and statutory requirements.

Contribute to multi-disciplinary reviews and attend Best Interest Meetings, supporting service users to achieve maximum independence and wellbeing.

Maintain registration with Social Work England and engage in regular supervision, training, and CPD to uphold professional standards and promote continuous improvement.
