Join Wokingham Borough Council’s Adult Social Work Team and help empower adults to live independently and with dignity. As a Qualified Social Worker, you’ll work in a strengths-based, person-centred environment, supporting individuals with complex needs while promoting their wellbeing and autonomy.

Key Responsibilities:

  • Conduct strengths-based assessments to determine eligibility for care and support under the Care Act 2014, focusing on individual capabilities and community resources.
  • Develop and review care and support plans in collaboration with service users, carers, and multi-agency partners to ensure outcomes are met effectively.
  • Safeguard vulnerable adults by identifying risks, undertaking Section 42 enquiries, and working closely with the Adult Safeguarding Hub.
  • Support hospital discharges and transitions through the Hospital Liaison and First Contact Teams, ensuring timely and safe care arrangements.
  • Maintain accurate case records and prepare reports in line with legal and professional standards, including Mental Capacity Act assessments where appropriate.
  • Engage in continuous professional development, with access to training such as Best Interests Assessor (BIA), AMCP, and Practice Educator qualifications.
