As a Physiotherapist with County Durham and Darlington NHS Foundation Trust, you will provide high-quality physiotherapy assessment, treatment and rehabilitation to support patients in achieving optimal physical function and independence. Working collaboratively with multidisciplinary teams, you will deliver evidence-based interventions tailored to individual needs over a 35-hour working week within the fixed 3-month period.
- 
Conduct thorough physiotherapy assessments to determine individual patient needs and goals. 
- 
Develop, implement and monitor treatment plans aimed at improving mobility, strength and function. 
- 
Work closely with patients, carers and healthcare professionals to ensure coordinated care delivery. 
- 
Maintain accurate and timely patient records in accordance with Trust and professional standards. 
- 
Provide education and advice to patients and carers to support ongoing self-management. 
- 
Contribute to service efficiency and quality improvement during the contract period.
