As an experienced and qualified Occupational Therapist with Leeds Community Healthcare NHS Trust, working at Band 6 within community services, you will provide specialist occupational therapy assessment and intervention focusing on equipment provision, adaptations and neurology. Collaborating closely with service users, families, carers and multidisciplinary teams, you will develop and deliver personalised programmes to support independence and enhance quality of life.

  • Conduct comprehensive assessments of functional abilities with a neurological focus and equipment needs.

  • Develop, implement and review individualised occupational therapy intervention plans centred on equipment, adaptations and neurological rehabilitation.

  • Work collaboratively with service users, carers and professionals to coordinate care and facilitate access to necessary equipment and adaptations.

  • Maintain accurate and up-to-date clinical records in line with NHS and professional standards.

  • Provide education, advice and training to support patients, carers and colleagues in neurological rehabilitation and equipment use.

  • Contribute to service development and quality improvement initiatives within the occupational therapy team.
