Job details: Assessment of new referrals to the service Identification of needs related to access, safety and independence in home environment. Support of junior staff in SSOT section of team. Completion of onward referrals as required; Completion of reports and attendance at Major Adaptations Panel as needed. The post holder will assess home environments and recommend equipment, adaptations or specify alternative housing needs. They will assess for and provide manual handling plans as needed. They will assess and recommend changes to packages of care. They will facilitate safe hospital discharges but providing timely home visits and equipment orders as needed. To have experience of supporting junior staff in developing skills. To be able to triage complex needs and use current team pathways. They will have extensive social services OT experience. Working hours: shifts, 09.00 – 17.00. Monday to Friday.
