We are seeking a dedicated and skilled Qualified Social Worker (QSW) to join our Adults’ Services team. This role involves working with adults, carers, and families to promote independence, uphold dignity, and ensure access to appropriate care through strengths-based social work practice.
Key Responsibilities:

Undertake strengths-based assessments: Work with adults to identify their needs, strengths, and outcomes, developing support plans that promote independence and well-being.

Safeguard vulnerable adults: Respond to concerns, carry out safeguarding enquiries, and implement protection plans in accordance with the Care Act 2014.

Work in a multidisciplinary context: Collaborate with health professionals, community services, and families to deliver integrated care and support.

Maintain professional standards: Ensure practice is compliant with legal frameworks, policies, and standards, and contribute to service improvement through reflective supervision and continuous development.
