This role involves working with adults, carers, and families to promote independence, uphold dignity, and ensure access to appropriate care through strengths-based social work practice.
Key Responsibilities:

Undertake strengths-based assessments: Work with adults to identify their needs, strengths, and outcomes, developing support plans that promote independence and well-being.

Safeguard vulnerable adults: Respond to concerns, carry out safeguarding enquiries, and implement protection plans in accordance with the Care Act 2014.

Work in a multidisciplinary context: Collaborate with health professionals, community services, and families to deliver integrated care and support.

Maintain professional standards: Ensure practice is compliant with legal frameworks, policies, and standards, and contribute to service improvement through reflective supervision and continuous development.
