Join the London Borough of Merton’s Adult Social Care team as a Community Care Social Worker, providing vital support to adults with care and support needs to promote independence and wellbeing.

Carry out statutory assessments, care planning, and reviews under the Care Act 2014 and Mental Capacity Act 2005, ensuring person-centred, strengths-based interventions.

Manage a varied caseload including safeguarding enquiries, hospital discharges, and support for adults with physical disabilities, mental health needs, and learning disabilities.

Work collaboratively with service users, carers, health professionals, housing, and voluntary organisations to deliver holistic, coordinated care and support.

Contribute to early intervention and prevention strategies that empower individuals and reduce the need for long-term care.
