Job Details:
Social Care Assessor required to support the discharge of clients from acute hospital settings in the Hospital Integrated Discharge Team. Suitable for an Occupational Therapy Assistant.

Responsibilities:

  • Complete strength-based, person-centred Care Act assessments.

  • Commission care/placements and liaise with clients, families, and other professionals/agencies.

  • Produce high-quality written work in a fast-paced environment.

  • Manage expectations of partner agencies to ensure safe and timely discharges.

  • Chair or participate in multi-disciplinary meetings as required.

  • Apply sound knowledge of issues affecting older people and people with disabilities.

  • Identify safeguarding concerns, complete enquiries, and apply safeguarding legislation.

  • Conduct Mental Capacity assessments, make best interest decisions, and apply the Care Act and Human Rights Act appropriately.

Skills/Experience:

  • Experience in Adults Social Care.

  • Strong interpersonal and communication skills for managing complex and sometimes challenging situations.

  • In-depth understanding of the legislative framework for Adult Social Care, including Safeguarding Adults, the Care Act, the Mental Capacity Act, and Human Rights Act.

Working Hours: Full time, Monday to Friday.
