Nottingham City Council is seeking a dedicated and experienced Social Worker to join the Learning Disabilities Team. The role focuses on working with adults with learning disabilities, promoting independence and ensuring their rights and wellbeing are upheld under the Care Act 2014 and Mental Capacity Act 2005.
Key Responsibilities:

Undertake Care Act assessments and reviews to develop person-centred care and support plans.

Carry out Mental Capacity Assessments and contribute to Best Interest Decisions.

Work with adults with learning disabilities and their families/carers to promote autonomy, safeguarding, and positive outcomes.

Liaise with health professionals, housing services, and community providers to ensure coordinated support.

Maintain accurate records and documentation in accordance with statutory requirements and council policy.
