Job Title: Occupational Therapist – Band 6

Job details:

 

Band 6 Occupational Therapist required to join the Community Rehabilitation Services Stroke Team.
This is an urgent requirement to start as soon as possible for an initial 2-month period.
The post holder will provide specialist Occupational Therapy assessments and interventions in a community setting
to adults with a diagnosis of stroke, working within a multidisciplinary team to deliver goal-focused rehabilitation.

 

  •  
  • Provide specialist Occupational Therapy assessment and treatment for stroke patients in the community
     

  • Manage a community-based caseload, delivering goal-focused neuro rehabilitation
     

  • Work collaboratively within a multidisciplinary team to support recovery and independence
     

  • Develop, implement and review individualised rehabilitation plans
     

  • Contribute to effective discharge planning and ongoing community support

 

Essential skills & experience:

 

  •  
  • HCPC registration
     

  • Neuro rehabilitation experience
     

  • Experience working in a community setting

 

Desirable skills:

 

  •  
  • Further experience within community rehabilitation services

 

Working hours: 30 hours per week (4 days). Monday to Friday, 08.30 – 16.30. Days flexible.
