As an experienced and qualified Occupational Therapist with Liverpool University Hospitals NHS Foundation Trust (DMS), working at Band 6 within community services, you will provide specialist occupational therapy assessment and intervention for adults with learning disabilities. Focusing on equipment provision and environmental adaptations, you will collaborate closely with service users, families, carers and multidisciplinary teams to promote independence and improve quality of life.

  • Conduct thorough assessments of adults’ functional abilities and learning disability-related needs.

  • Develop, implement and review personalised occupational therapy intervention plans centred on equipment and adaptations.

  • Work collaboratively with service users, carers and professionals to coordinate effective care and access to community resources.

  • Maintain accurate and up-to-date clinical records in accordance with NHS and professional standards.

  • Provide education, advice and training to support service users, carers and colleagues in equipment use and adaptations.

  • Contribute to service development and quality improvement initiatives within the occupational therapy team.
