As an Occupational Therapist at County Durham and Darlington NHS Foundation Trust, you will support patients to regain independence and improve their quality of life through personalized therapeutic interventions. Working within a multidisciplinary team, you will assess, plan, and deliver treatment programs tailored to meet the physical, cognitive, and emotional needs of diverse patient groups.

Key Responsibilities:

  • Assess patients’ functional abilities and rehabilitation needs using a range of standardized tools and clinical judgment.

  • Design and implement individualized treatment plans aimed at promoting independence in daily living activities.

  • Advise on and provide assistive devices, environmental adaptations, and therapeutic equipment to support patient goals.

  • Work collaboratively with healthcare professionals, patients, and families to ensure holistic and coordinated care.

  • Monitor patient progress, evaluate outcomes, and adjust interventions accordingly to optimize recovery.

  • Maintain accurate documentation and contribute to service development, audit, and quality improvement initiatives.
