Occupational Therapist

 

Job Overview

Dorset Council is seeking a motivated and experienced Occupational Therapist to join its expanding Home First Team. This hybrid locum role is focused on supporting timely hospital discharges, recovery, and reablement, helping residents across Dorchester, Ferndown, and Sherborne regain independence and live safely at home.  

 

As part of the wider Home First Social Care Team, you’ll work collaboratively across health and community services to deliver coordinated, person-centred care. You’ll help ensure people receive the right support at the right time, improving health outcomes and reducing avoidable hospital admissions.

 

Key Responsibilities

– Undertake comprehensive assessments to identify individuals’ functional needs and develop personalised care and reablement plans.  

– Support safe and timely hospital discharges through the Home First approach.  

– Work collaboratively with multidisciplinary teams, including social workers, nurses, and therapists, to promote recovery and independence.  

– Recommend and arrange equipment or home adaptations to enhance daily living and mobility.  

– Deliver interventions based on recognised occupational therapy models and evidence-based practice.  

– Travel across locality clusters to ensure consistent and joined-up service delivery.  

– Maintain clear, accurate, and timely documentation in accordance with HCPC and council standards.

You’ll be a compassionate and proactive Occupational Therapist who thrives in a fast-paced, multidisciplinary environment. With strong assessment skills and a solution-focused approach, you’ll support people to achieve their goals and maintain independence at home.
