Job Title:  Rehab Support Worker – Band 3  (2 roles)
Area: East London NHS

Contract length: Ongoing 
Job details: To start asap for 6 weeks. Reporting back to the case holder of any concerns noted/rehabilitation progress/ review of client goals. The post holder will support the daily observation taking and identify any concerns or change in patterns, supporting the prevention of admission into hospital. They will attend weekly multi-disciplinary meetings to provide this feedback and support the progression of the clients.
1. Is responsible for implementing care and rehab plans written by, for instance, Physiotherapists, Occupational Therapists.
2. Provides regular feedback to the leads and team to allow them to review and adapt rehab/ care plans accordingly. 3. Contributes to client goal planning sessions and shows initiative in areas where competent and as agreed by senior staff.
4. Assists in helping to prevent unnecessary hospital admission, promotes timely and safe discharge, proactive case management and client self-care of chronic conditions.
5. Ensures accurate record keeping and carries out admin tasks.
6. To work autonomously and undertake lone working in the community.
7. Where barriers to understanding exist, staff will need to use tact, motivational, negotiating and reassurance skills. Information received and given may be sensitive and will need to be handled in an empathic and professional manner maintaining confidentiality within Trust Policies and professional codes of conduct.
8. To involve clients and carers in the planning of care/rehabilitation programmes and encourage self-management where appropriate.
9. To communicate effectively with all members of the Integrated Community Service as well as the wider    Multidisciplinary Team in order to support and promote team working.
