Job details: Occupational Therapist/ Physiotherapist required for 2-hour Urgent Community Response. To start asap.
Experience of community working.
Experience of Rapid Response work would be preferred.
Own a car and have a valid driving license to be able to drive within Preston, Chorley and South Ribble areas.
Experience of working with adults and older adults with a range of health conditions, including frailty and acute medical conditions.
Experience of working with vulnerable patients and those who lack capacity.
Experience of providing equipment such as walking aids, stand aids, hoists, profiling beds, commodes.
Ability to undertake moving and handling and mobility assessments.
Ability to manage a varied team caseload.
Ability to work as part of a multi-disciplinary team.
Ability to manage and lead on difficult/ complex situations.
Ability to prioritise caseload throughout the day and identify team capacity.
Ability to work within a fast-paced team.
Ability to delegate appropriate tasks to band 4.
Ability to provide clinical support to colleagues.
Ability to communicate clinical reasoning verbally to patients and professionals and within clinical notes.
Ability to use electronic clinical systems.
Working hours: Full time or part-time. Monday to Friday between the hours of 08:00-20:00, flexibility for long days and possible weekends.
