- Deliver Discharge to Assess (D2A) Occupational Therapy interventions
- Work collaboratively with health and social care colleagues to support safe and timely discharges
- Undertake assessments and provide recommendations to promote independence
- Operate within a hybrid working model, balancing remote and on-site work
Working hours: 37 hours per week. Monday to Friday, 09.00 – 17.30.
