Job Overview
We’re recruiting an experienced Community Physiotherapist to support the Reablement service at Bexley Council.
This role focuses on short-term, goal-driven rehabilitation that helps people regain independence, reduce reliance on long-term care, and return safely to everyday activities following illness, injury, or a hospital stay.
You’ll work within the community, carrying out functional assessments, delivering rehabilitation programmes, and collaborating closely with colleagues across health and social care to support safe and timely recovery.
Key Responsibilities
- Deliver person-centred reablement physiotherapy in service users’ homes and community settings.
- Complete holistic assessments and set realistic, measurable rehabilitation goals.
- Design and monitor therapy plans that promote mobility, confidence, and independence.
- Work collaboratively with occupational therapists, support workers, and social care teams.
- Provide advice on safe movement, falls prevention, and mobility equipment where appropriate.
- Review progress regularly and adjust treatment plans in line with service users’ needs.
- Maintain accurate records in accordance with professional and service requirements.
About You
You are a proactive and compassionate physiotherapist who enjoys community-based practice and working closely with people in their own homes. You’re confident planning and prioritising your caseload and committed to supporting people to live as independently as possible.
Requirements
- Recognised Physiotherapy qualification and HCPC registration.
- Previous experience in community, reablement, or rehabilitation-focused services.
- Good understanding of strength-based, short-term intervention approaches.
- Effective communication and clinical reasoning skills.
- Ability to work autonomously and as part of a multi-disciplinary team.
- Car driver preferred (community travel may be required).
Contract & Salary
- Locum position – ongoing
- Up to £36 per hour (depending on experience)
- Community-based reablement caseload
