View JobWe are seeking an experienced Social Worker to join Cornwall Council’s Adults Community Team on a 3-month locum contract. This role will involve working with adults in the community to assess needs, plan and implement care packages, and ensure the delivery of person-centred support in line with statutory requirements. The successful candidate will be committed to promoting independence, safeguarding vulnerable adults, and working collaboratively with multi-disciplinary teams.
Key Responsibilities:

Undertake Care Act assessments to identify individual needs and determine appropriate support plans.

Carry out Mental Capacity Act assessments and participate in Best Interest decision-making where required.

Manage a caseload of adults in the community, ensuring timely interventions and reviews of care packages.

Work collaboratively with health professionals, voluntary agencies, and other stakeholders to deliver coordinated care.

Identify and respond to safeguarding concerns in line with local authority policies and procedures.
-
Maintain accurate and up-to-date case records in compliance with legislative and organisational requirements.
