About the Role

 

Brent Council is seeking an experienced Care Assessor to join the Learning Disabilities Support Planning Team. This role involves delivering high-quality assessments and support planning to ensure individuals receive appropriate, person-centred care and support.

 

Main Responsibilities

 

  • Carry out social care and joint health assessments.
  • Develop comprehensive, outcomes-focused support plans to meet identified needs.
  • Evaluate and identify appropriate care options, including traditional care, reablement and supported living.
  • Participate in care discussions and multidisciplinary assessments.
  • Adopt a creative and flexible approach to identifying suitable services.
  • Research and utilise both formal and informal services to meet individual needs.
  • Monitor and review packages of care to ensure they remain appropriate and effective.
  • Work in partnership with statutory, voluntary and private sector organisations.
  • Contribute to the ongoing development of the team.
  • Undertake safeguarding responsibilities in line with local Adult Safeguarding policies.

 

Skills & Experience Required

 

  • Experience working within adult social care, ideally with learning disabilities.
  • Strong assessment, support planning and review skills.
  • Experience of multidisciplinary and partnership working.
  • Good understanding of safeguarding adults procedures.
  • Excellent communication, organisational and problem-solving skills.

 

An excellent opportunity to contribute to a specialist learning disabilities service within a supportive local authority team.
