Job Details:
Derbyshire County Council is seeking enthusiastic and motivated Community Care Workers to provide occupational therapy support within the Adult Care Team. The role involves supporting adults in the community through assessments, equipment solutions, and minor/major adaptations, working in partnership with housing providers and other agencies. The post holder will be supported by Occupational Therapists, Senior Practitioners, and Team Coordinators.

Key Responsibilities:

  • Conduct routine home visits and assessments under the direction of a Service Manager.

  • Provide equipment and adaptation solutions to meet assessed needs.

  • Take a strength-based, person-centered, holistic approach to care.

  • Maintain accurate and timely records; use IT systems effectively (Mosaic desirable).

  • Collaborate with multi-disciplinary teams and external partners.

  • Adhere to statutory legislation and safeguarding principles in adult social care and health.

  • Manage and prioritise caseload effectively.

Skills/Experience Required:

  • Experience working with adults in a community/Local Authority setting.

  • Knowledge of activities of daily living, physical, learning disabilities, and/or mental health conditions.

  • Experience with equipment and adaptations, including minor and major works.

  • Excellent communication, time management, and IT skills.

  • Ability to travel for home visits and manage caseload autonomously.

  • Reflective practitioner with commitment to professional development and supervision.
