You will collaborate closely with multidisciplinary colleagues to deliver effective, person-centred care.

Undertake comprehensive occupational therapy assessments to determine individual needs.

Recommend and arrange suitable equipment and adaptations to promote independence and safety.

Provide professional advice to service users, families and carers on safe use of equipment.

Work collaboratively with multidisciplinary teams to coordinate care and achieve agreed outcomes.

Monitor and review interventions to ensure effectiveness and adjust plans as required.

Maintain accurate and timely documentation in line with professional and organisational standards.
