Conduct thorough assessments of functional abilities and housing-related needs.

Develop, implement and review personalised intervention plans to support independent living.

Work closely with service users, carers and professionals to coordinate care and access to community resources.

Maintain accurate and up-to-date clinical records in line with NHS and professional standards.

Provide advice, education and training on adaptive equipment and environmental modifications.

Contribute to service development and improvement initiatives within housing and community occupational therapy services.
