Sussex Community NHS Foundation Trust (SCFT) is looking for a compassionate and motivated Occupational Therapist to join its dedicated team. As the largest provider of community healthcare in Sussex, SCFT delivers high-quality care that supports individuals to live independently and safely within their communities. This role offers the opportunity to make a meaningful difference to patients’ lives, while working within a supportive, forward-thinking environment that values development and innovation.

Key Responsibilities:

  • Provide high-quality, evidence-based occupational therapy assessments and interventions across community settings, including patients’ homes, clinics, and rehabilitation units.

  • Work collaboratively within a multi-disciplinary team, including physiotherapists, nurses, social workers, and other healthcare professionals, to ensure holistic patient care.

  • Develop and implement personalised treatment plans to support patients in achieving their functional goals and improving their quality of life.

  • Contribute to safe and timely discharge planning by promoting independence in daily living activities and reducing the risk of hospital readmission.

  • Maintain accurate and timely clinical documentation in line with Trust policies and professional standards.

  • Participate in ongoing professional development, supervision, and training opportunities, and contribute to service improvement initiatives.
