Key Responsibilities:

  • Develop and implement care plans: Undertake assessments and coordinate child-centred care plans in partnership with children, families, and carers.

  • Monitor progress and outcomes: Regularly review care plans to ensure they meet the evolving needs of the child and adjust as necessary.

  • Collaborate with multi-agency partners: Work closely with health, education, legal, and voluntary sectors to ensure comprehensive support.

  • Maintain detailed records: Document assessments, plans, and reviews accurately, ensuring compliance with statutory and council requirements.
