View JobWe are seeking a dedicated and experienced Qualified Social Worker to join Wokingham Borough Council’s Adult Social Care team. This role focuses on delivering high-quality care planning and support to adults with a range of needs, promoting independence and well-being in line with the Care Act 2014. You will be working within a supportive and collaborative environment committed to strengths-based practice and person-centred care.
Key Responsibilities

Undertake strength-based assessments under the Care Act 2014 to identify eligible needs and co-produce care and support plans with individuals and carers.

Coordinate and review care packages to ensure they remain effective, person-centred, and aligned with outcomes, budget, and legislative requirements.

Work in partnership with health professionals, housing teams, and voluntary sector services to deliver integrated support for adults with complex needs.

Safeguard vulnerable adults by responding to concerns, carrying out Section 42 enquiries, and following local safeguarding procedures.

Maintain accurate and timely case recordings, assessments, and documentation on the council’s social care system (e.g., Mosaic).

Engage in reflective supervision, contribute to team meetings, and maintain professional development in line with Social Work England standards.
