About the Role

 

Salisbury District Hospital is seeking an experienced and proactive Band 6 Physiotherapist to join a developing service model working seamlessly across acute and community settings. This role focuses on supporting safe, timely and effective discharge for patients who are medically fit, enabling rehabilitation and independence at home.

 

Main Responsibilities

 

  • Carry out comprehensive physiotherapy assessments for patients preparing for discharge.
  • Identify care needs, set patient-centred goals and develop rehabilitation plans to support safe transition home.
  • Assess, recommend and order appropriate equipment to facilitate independence and safety.
  • Work collaboratively within the MDT and across provider boundaries to ensure continuity of care.
  • Deliver rehabilitation across both acute and community environments as part of an integrated service model.
  • Maintain accurate and timely clinical documentation in line with HCPC and NHS standards.
  • Adopt a flexible, proactive and positive approach when responding to service needs.

 

Requirements

 

  • HCPC-registered Physiotherapist.
  • Experience in both acute and community physiotherapy settings.
  • Strong assessment, goal-setting and clinical reasoning skills.
  • Competence in equipment assessment and prescription.
  • Excellent MDT working, communication and organisational skills.
  • A proactive, solution-focused and “can-do” attitude.

 

Working Hours

 

  • Core hours with potential weekend working as required.
