Reablement Assistant (Locum)

 

Location: Liverpool City Council – Granby Hub

About the Role

 

The Granby Hub is a 30-bed Intermediate Care and Reablement service in central Liverpool, supporting adults to regain and sustain their independence. We are seeking a committed Reablement Assistant to deliver person-centred care and practical support, working closely with families, professionals and wider agencies to ensure coordinated, high-quality rehabilitation.

 

Main Responsibilities

 

  • Support the physical, social and emotional needs of service users in line with their care plans, promoting independence and life skills.
  • Assist with daily living tasks as agreed in support plans, using appropriate equipment where required.
  • Encourage individuals to improve and maintain essential skills for independent or assisted living, including kitchen tasks, household routines, personal care and health management.
  • Work collaboratively with families, allied health professionals and partner agencies to minimise risk and support individuals to remain in their own homes.
  • Contribute to person-centred support plans and participate in assessments and reviews.
  • Administer medication when required (for community-based staff).
  • Promote dignity, empowerment and respect for cultural, spiritual and religious needs.
  • Hold keys to service users’ homes where needed and ensure correct security procedures are followed.
  • Maintain accurate records and report any changes in service user wellbeing to the Line Manager.
  • Attend regular supervision and participate in staff training and development.
  • Work flexibly as part of a 7-day service between 08:00 and 22:00, adapting to changing service needs.
  • Support service users in a range of activities, including recreational, therapeutic and community-based tasks such as hydrotherapy and rebound therapy.
  • Provide hands-on support within the onsite café, including periods of prolonged standing when required.
  • Ensure all work aligns with Council policies, including equal opportunities, anti-discriminatory practice and health and safety.
  • Contribute to the Council’s social value commitments by using resources responsibly.
  • Deliver excellent customer service and support continuous improvement across the service.

 

Skills & Experience

 

  • Essential: NVQ Level 3 in Health and Social Care (or willingness to work towards qualification).
  • Experience working in an adult social care environment.
  • Experience supporting individuals to develop and maintain independence.
  • Desirable: Manual Handling certificate, experience delivering person-centred support.
  • Strong communication skills and ability to build rapport with a range of people.
  • Reliable and flexible approach to working across multiple shift patterns.

 

Working Hours

 

  • Flexible rota between 08:00 and 22:00, across 7 days per week.
