About the Role

 

Lewisham and Greenwich NHS Trust is seeking an experienced Band 6 Occupational Therapist to join their Community Therapies Service. This role focuses on supporting adults in their homes through comprehensive assessment, rehabilitation and preventative interventions aimed at improving function and reducing hospital admissions.

 

Main Responsibilities

 

  • Undertake holistic occupational therapy assessments within patients’ homes, including functional and cognitive assessments.
  • Recommend and provide appropriate equipment to enhance safety, independence and day-to-day functioning.
  • Develop and deliver person-centred treatment plans to support rehabilitation and prevent deterioration.
  • Work collaboratively within a multidisciplinary team, including physiotherapists, therapy assistants and social workers.
  • Promote safe, effective and timely interventions to reduce the risk of hospital admissions.
  • Maintain accurate and timely clinical documentation in line with Trust and HCPC requirements.

 

Requirements

 

  • HCPC-registered Occupational Therapist.
  • Experience in community-based occupational therapy.
  • Strong assessment and clinical reasoning skills, including functional and cognitive assessment.
  • Knowledge of equipment provision and community rehabilitation pathways.
  • Excellent communication and MDT working skills.

 

Working Hours

 

  • 37.5 hours per week.
  • Monday to Friday, 08:30 – 16:30.
