Job details: Community services. • Have experience of working with older adults. • To work flexible hours to meet the needs of patients and their families. • To provide high quality patient-centred care and specialist rehabilitation to adults referred to the Rehabilitation Service. • Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments. • To provide advice regarding the rehabilitation management of patients to families, carers, and other members of the MDT. • Responsibility for a caseload of patients within both the community and / or the community hospital bedded environment. • Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the teams. • Alongside Senior clinical staff and Clinical Educator, give support, guidance and training to Senior Rehabilitation Assistants, Rehabilitation Assistants, and students on placement within the team. • To be responsible for organising, prioritising, and planning your own caseload to meet service and patient priorities, readjusting plans as situations change/arise. • To ensure that your own practice, and that of the staff under your supervision, meets the required professional standards of Physiotherapy. • To ensure your own ongoing personal/professional development. • To comply with the Trust and departmental policies and procedures • To adhere to your code of conduct.
Working hours: shifts, 09.00 – 17.00. Part time.
