Job details:

 

Experienced and dynamic Occupational Therapist required for the Independent Living Service. The service works across Herefordshire communities to support adults of all ages and medical conditions to achieve independence in meaningful daily activities and improve quality of life.

 

  •  
    • Work collaboratively with social care, health, housing professionals and external agencies
       

    • Undertake statutory, strengths-based Occupational Therapy assessments in line with legislation
       

    • Identify risks and barriers to independence and apply prevention and enablement strategies
       

    • Complete functional, social and environmental needs assessments through observation and therapeutic engagement
       

    • Create meaningful, personalised programmes to support access to services and promote independence
       

    • Work autonomously using a holistic, person-centred approach to promote meaningful occupation
       

    • Deliver early intervention to reduce, delay or prevent escalation of care and support needs
       

    • Provide bespoke interventions, information and advice within people’s homes and local environments
       

    • Facilitate provision of aids to daily living, adaptations, moving and handling solutions, specialist community equipment and technology enabled living
       

    • Maintain a varied caseload and deliver, review and evaluate Occupational Therapy interventions
       

    • Support the supervision and development of junior staff
       

    • Maintain professional standards and contribute to continuous service improvement

     

    Working arrangements:

     

    •  
      • Flexible working available, including job share, part-time hours and agile working
         

      • Full time: 37 hours per week
