About the Role
The London Borough of Harrow is seeking an Occupational Therapy Assistant or Trusted Assessor to provide a professional fieldworker service supporting adults with physical and sensory disabilities, including those with terminal illness. The post holder will carry out assessments, recommend equipment and minor adaptations, and promote independence in line with Care Act responsibilities and local authority policy.
This community-based role requires regular on-site presence in Harrow (minimum 3 days per week) and the ability to install low-level equipment during assessments.
Main Responsibilities
- Provide person-centred support to adults ordinarily resident in Harrow, ensuring timely and appropriate interventions.
- Undertake assessments of need, reviewing strengths, risks, and opportunities to promote independence in daily living.
- Recommend and arrange provision of small equipment, minor adaptations and other support services.
- Work across Harrow’s community settings, including Community Hubs and home environments.
- Complete Care Act 2014 assessments and reviews, supporting individuals to identify goals, outcomes and wellbeing priorities.
- Champion the needs of individuals with specialist or complex needs, promoting independence and resilience.
- Collaborate with colleagues across health, social care and partner organisations, contributing to innovative practice and community engagement.
- Provide accurate advice, information and signposting to community resources and support networks.
- Maintain high-quality case recording and ensure services are delivered within available resources.
Skills & Experience Required
- Previous experience working in Adult Social Care or a community setting.
- Understanding of equipment provision, adaptations and promoting independence.
- Ability to undertake assessments and contribute to care planning under the Care Act framework.
- Physically able to transport and install low-level equipment for assessment purposes.
- Car driver with access to a vehicle for work purposes.
- Strong communication, problem-solving and organisational skills.
- Ability to work across multiple locations within Harrow.
Working Hours
- 36 hours per week.
- Must be able to attend on-site in Harrow at least 3 days per week.
