Introduction:
We are seeking a compassionate and experienced Adults Social Worker to join our dedicated team. This role involves working with vulnerable adults to assess needs, promote independence, and ensure access to the appropriate services and support. The ideal candidate will demonstrate empathy, professionalism, and a strong commitment to safeguarding adults at risk.
Key Responsibilities:

Conduct assessments and care planning for adults with complex needs, ensuring compliance with the Care Act and other relevant legislation.

Work collaboratively with families, health professionals, and community partners to coordinate and deliver effective care packages and interventions.

Safeguard vulnerable adults by identifying risks, managing protection plans, and responding to safeguarding concerns in line with statutory procedures.

Maintain accurate, timely case records and reports, using digital systems to document assessments, reviews, and outcomes.
