View JobAs an Occupational Therapist with Medway Council, working on a three-month contract, you will provide specialist assessment and intervention to support individuals in maintaining independence, safety and wellbeing. Working collaboratively with service users, families, carers and multidisciplinary teams, you will deliver tailored solutions, equipment provision and adaptations to meet individual needs.

Carry out comprehensive assessments of functional abilities and daily living needs in home and community settings.

Recommend, arrange and review appropriate equipment and adaptations to promote independence.

Develop and implement person-centred plans in line with assessed needs and best practice.

Liaise with service users, carers and other professionals to ensure coordinated support.

Maintain accurate and up-to-date case records in accordance with council and professional standards.

Contribute to service improvement initiatives within the occupational therapy team.
