Job Title: Reablement Officer

 

Location: London Borough of Newham

Area: East London Social Services

Salary: £negotiable

Contract Length: Ongoing (initially 3 months)

 

Service: Adult Reablement Team

 

Job Details:

 

  •  
    • Reablement Officer required to start ASAP
       

    • Deliver short-term reablement interventions to adults to maximise independence at home
       

    • Support individuals to regain skills and confidence, reducing or preventing ongoing care needs
       

    • Work as part of an integrated intermediate care/reablement multidisciplinary team
       

    • Contribute to person-centred care and support planning documentation
       

    • Maintain accurate and timely records in line with service and statutory requirements
       

    • Car driver preferred for community visits
       

    • Current hybrid working model – 2–3 days per week for visits, remainder home-based as appropriate

     

    Essential Skills/Experience:

     

    •  
      • Experience in Adult Social Care Reablement and/or Rehabilitation
         

      • Experience working effectively within a Multidisciplinary Team (MDT)
         

      • Strong communication skills and ability to work collaboratively across professional boundaries
         

      • Experience contributing to clear, accurate, person-centred care and support plans
         

      • Confident using digital systems (health and/or social care platforms desirable)

       

      Working Hours: Monday to Friday.
