Service: Adult Social Care – Major Adaptations
Job Details:
- Occupational Therapist required to support Adult Social Care services focusing on major adaptations.
- Carry out occupational therapy assessments for adults with physical disabilities and complex needs.
- Assess and recommend major housing adaptations to support independence and safety at home.
- Work closely with housing services, surveyors and multidisciplinary teams to implement adaptation solutions.
- Support service users to remain safely in their homes and reduce the need for long-term care packages.
- Maintain accurate case records and documentation in line with service standards.
Skills / Experience:
- Qualified Occupational Therapist with HCPC registration.
- Experience within Adult Social Care.
- Experience assessing for major adaptations and housing solutions.
- Knowledge of equipment provision and moving and handling.
- Ability to manage a caseload independently.
- Strong communication and multidisciplinary working skills.
