Service: Adult Reablement Team
Job Details:

- Reablement Officer required to start ASAP
- Deliver short-term reablement interventions to adults to maximise independence at home
- Support individuals to regain skills and confidence, reducing or preventing ongoing care needs
- Work as part of an integrated intermediate care/reablement multidisciplinary team
- Contribute to person-centred care and support planning documentation
- Maintain accurate and timely records in line with service and statutory requirements
- Car driver preferred for community visits
- Current hybrid working model – 2–3 days per week for visits, remainder home-based as appropriate
Essential Skills/Experience:

- Experience in Adult Social Care Reablement and/or Rehabilitation
- Experience working effectively within a Multidisciplinary Team (MDT)
- Strong communication skills and ability to work collaboratively across professional boundaries
- Experience contributing to clear, accurate, person-centred care and support plans
- Confident using digital systems (health and/or social care platforms desirable)
Working Hours: Monday to Friday.
