Tripod Partners are recruiting a Community Occupational Therapist for a locum role within West Yorkshire Social Services. This position is based within the Recovery Hubs, working alongside multidisciplinary colleagues to promote independence, rehabilitation, and safe discharge home.
The Occupational Therapist will support adults with a range of needs through strengths-based assessments, goal-led intervention planning, rehabilitation, and discharge coordination. The role forms part of a rehabilitation-focused service that promotes a 24-hour therapeutic approach across health and social care teams.
Key Responsibilities
- Carry out occupational therapy assessments using a strengths-based and personalised approach.
- Develop and implement goal-led rehabilitation and outcome plans.
- Support individuals to maximise independence and return safely to their own homes.
- Lead discharge planning for individuals returning home from Recovery Hubs.
- Work collaboratively with Physiotherapists, Nurses, support workers, and wider multidisciplinary teams.
- Review progress regularly and adapt intervention plans where appropriate.
- Undertake moving and handling assessments and develop risk management solutions.
- Support disabled individuals with interventions that balance safety, independence, and personal outcomes.
- Maintain accurate and timely records in accordance with statutory and organisational standards.
- Provide a high-quality occupational therapy service to adults, children, young people, carers, and the wider community.
About You
You will be a qualified Occupational Therapist with experience working within
community rehabilitation or social care services. You will demonstrate strong assessment
and clinical reasoning skills alongside a commitment to strengths-based and person-centred practice.
You will be confident working within multidisciplinary teams and supporting rehabilitation, discharge
planning, and independence-focused interventions in community settings.
Requirements
- Recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Experience within community rehabilitation or adult social care services.
- Strong assessment, rehabilitation, and discharge planning skills.
- Experience with moving and handling assessments and risk management.
- Ability to develop and implement goal-led intervention plans.
- Strong communication, organisational, and multidisciplinary working skills.
