Tripod Partners are recruiting a Reablement Occupational Therapist for a locum role within East London Social Services. This position supports a reablement project within Hackney Council, helping adults regain independence and confidence within their own homes and community settings.
The Occupational Therapist will work within a multidisciplinary environment, supporting service users through strengths-based assessments, reablement interventions, and person-centred goal planning to maximise independence and wellbeing.
Key Responsibilities
- Carry out occupational therapy assessments within community and home settings.
- Support adults through reablement-focused interventions and rehabilitation plans.
- Promote independence, confidence, and safe living within the community.
- Develop person-centred goals and support plans.
- Work collaboratively with multidisciplinary teams and partner agencies.
- Support discharge and reablement pathways where appropriate.
- Maintain accurate and timely clinical and case records.
- Provide advice, guidance, and support to service users and carers.
About You
You will be a qualified Occupational Therapist with experience working within
reablement, community, or adult social care services. You will demonstrate strong
assessment and clinical reasoning skills alongside a person-centred and strengths-based approach.
You will be confident managing community caseloads and supporting individuals to maximise independence
and achieve positive rehabilitation outcomes.
Requirements
- Recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Experience within reablement, community rehabilitation, or adult social care.
- Strong communication and organisational skills.
- Ability to work effectively within multidisciplinary teams.
- Experience supporting independence-focused interventions desirable.
