Job Overview
We are recruiting an experienced **Advanced Practitioner (Qualified Social Worker)** to join **Stoke-on-Trent City Council’s Adults’ Services team**. This senior role combines complex casework with leadership responsibilities, providing expert practice guidance to colleagues and ensuring that adults receive high-quality, person-centred care and safeguarding support.
As an Advanced Practitioner, you will work with individuals, families, and partner agencies to promote independence, wellbeing, and dignity, while also playing a key role in service development and mentoring within the team.
Key Responsibilities
– Manage and lead on complex adult social care cases, including safeguarding and Mental Capacity Act assessments. – Provide professional guidance, reflective supervision, and mentoring to social workers and support staff. – Develop, implement, and review personalised care and support plans in partnership with adults and their families. – Contribute to court work, best-interest decisions, and statutory reporting where required. – Work collaboratively with health professionals, housing, voluntary sector, and community services to deliver integrated care. – Ensure compliance with the Care Act 2014, Mental Health Act, and other relevant legislation. – Support service development, audits, and quality assurance processes within Adults’ Services. – Promote professional standards, continuous improvement, and evidence-based social work practice.
About You
You are a confident and resilient social worker with strong leadership qualities and advanced practice skills. You are committed to safeguarding adults, championing their rights, and enabling them to live as independently as possible. With excellent interpersonal and analytical abilities, you can balance complex decision-making with mentoring and supporting your colleagues.
Requirements
– Qualified Social Worker (QSW) with registration to **Social Work England**. – Significant post-qualification experience within **Adults’ Services**, including complex case management. – In-depth knowledge of adult safeguarding, the Care Act 2014, and related legislation. – Demonstrable experience of mentoring, supervising, or leading other practitioners. – Excellent communication, negotiation, and report-writing skills. – Strong ability to work under pressure and manage competing priorities.
