Key Responsibilities:

Undertake Care Act assessments and person-centred support planning, ensuring adults receive the appropriate care and support to meet their assessed needs.

Safeguard vulnerable adults by leading and contributing to Section 42 enquiries and promoting safety and well-being through risk management strategies.

Work in partnership with families, carers, health services, and voluntary organisations to deliver integrated, outcome-focused care.

Maintain clear, accurate case records and contribute to service development through reflective practice, supervision, and continuous learning.
