Key Responsibilities:

  • Undertake Care Act assessments and person-centred support planning, ensuring adults receive the appropriate care and support to meet their assessed needs.

  • Safeguard vulnerable adults by leading and contributing to Section 42 enquiries and promoting safety and well-being through risk management strategies.

  • Work in partnership with families, carers, health services, and voluntary organisations to deliver integrated, outcome-focused care.

  • Maintain clear, accurate case records and contribute to service development through reflective practice, supervision, and continuous learning.
