Carry out statutory assessments, care planning, and safeguarding enquiries under the Care Act 2014, Mental Capacity Act 2005, and related legislation.

Manage a dynamic caseload involving individuals with physical disabilities, mental health needs, learning disabilities, and older adults requiring support or intervention.

Work collaboratively with service users, carers, families, and professionals from health, housing, and voluntary sectors to deliver integrated, person-centred care.
