Carry out Care Act-compliant assessments on behalf of Gateshead Council, supporting adults to identify their strengths, achieve outcomes, and maintain independence within the community.

Manage a varied caseload involving older adults, people with physical or learning disabilities, and individuals with mental health needs, ensuring timely and proportionate interventions.

Work collaboratively with service users, carers, and multi-disciplinary teams to co-produce care and support plans that are person-centred, strengths-based, and cost-effective.

Undertake safeguarding enquiries in accordance with Section 42 of the Care Act 2014, contributing to the protection of vulnerable adults from abuse and neglect.

Maintain accurate records and prepare reports in line with legal and professional standards for panels, case conferences, court proceedings, and Continuing Health Care assessments.

Contribute to team development and continuous improvement by participating in supervision, reflective practice, audits, and professional learning opportunities within Gateshead Council.
