Lead on complex and high-risk cases involving safeguarding, mental capacity, hospital discharge, and Continuing Health Care (CHC), providing expert input and oversight in line with the Care Act 2014.

Act as a practice lead within the team, offering professional advice, coaching, and mentoring to social workers and newly qualified staff, and promoting strengths-based, person-centred practice.

Undertake and quality-assure complex assessments, support plans, and risk management strategies, ensuring compliance with legal frameworks including the Mental Capacity Act and Human Rights Act.

Chair multi-agency meetings and best interest decisions, promoting effective partnership working across health, housing, and voluntary sectors to meet the needs of adults and carers.

Contribute to service development and improvement, participating in audits, reflective sessions, and policy implementation to enhance practice standards and outcomes.

Support the team manager with operational duties, including supervision, performance monitoring, and the delivery of key service priorities within Adult Social Care.
