Sussex Community NHS Foundation Trust – Southfield House is seeking a qualified and experienced Band 6 Physiotherapist to join the HomeFirst team. This community-based role provides the opportunity to deliver physiotherapy to adults in their own homes, supporting safe discharge, recovery, and independence while working as part of a multidisciplinary service.

Undertake comprehensive physiotherapy assessments and deliver evidence-based interventions for adults within the HomeFirst service.

Support patients in regaining functional independence, mobility, and confidence at home following hospital discharge or acute illness.

Work collaboratively with the multidisciplinary team to ensure coordinated, patient-centred care.

Contribute to the planning and delivery of tailored treatment programmes, rehabilitation pathways, and safe discharge plans.

Provide guidance and support to junior physiotherapists, therapy assistants, and students.

Participate in service evaluation, audit, and quality improvement initiatives to maintain and enhance standards of physiotherapy practice.
