Undertake Care Act 2014 assessments on behalf of Staffordshire County Council, supporting adults with care and support needs to live independently and safely.

Manage a varied caseload including older adults, individuals with physical disabilities, learning disabilities, and mental health conditions, ensuring person-centred and strengths-based interventions.

Develop, implement, and review personalised care and support plans, working in partnership with service users, carers, and other professionals to promote wellbeing and independence.

Conduct safeguarding enquiries under Section 42 of the Care Act, coordinating multi-agency responses to protect adults at risk of abuse or neglect.

Maintain accurate and timely records and reports in line with statutory duties, including Mental Capacity Act assessments, best interest decisions, and Continuing Health Care processes.

Engage in regular supervision, reflective practice, and ongoing professional development, contributing to high-quality adult social care delivery within Staffordshire County Council.
