Complete Care Act 2014 assessments, support planning, and reviews to meet the needs of adults, older people, and carers using a strengths-based and person-centred approach.

Manage a diverse caseload, including safeguarding enquiries, hospital discharges, and complex cases involving mental capacity and best interest decisions.

Work collaboratively with health professionals, community services, housing, and voluntary organisations to deliver integrated and responsive care and support.

Record all casework clearly and accurately on CareDirector (or the council’s case management system), ensuring compliance with statutory and local procedures.

Engage in multi-agency meetings such as safeguarding conferences, best interest meetings, and case reviews to advocate for service users and promote positive outcomes.

Maintain professional registration with Social Work England and actively participate in supervision, training, and continuing professional development (CPD).
