Deliver statutory adult social care services for Bracknell Forest Borough Council, supporting individuals under the Care Act 2014 to achieve independence and improved wellbeing.

Carry out strengths-based assessments and reviews, developing personalised care and support plans in partnership with service users and carers.

Manage a caseload involving older people, adults with physical or learning disabilities, and those with mental health needs.

Undertake Section 42 safeguarding enquiries, working with multi-agency partners to identify and manage risks and ensure protection.

Apply the Mental Capacity Act 2005 in practice, completing capacity assessments and supporting best interest decisions when necessary.

Maintain accurate and up-to-date case records, participate in supervision, and contribute to team learning and service development initiatives.
