Undertake Care Act 2014 assessments, support planning, and reviews to identify eligible needs and promote independence for adults and older people.

Manage a diverse caseload including complex cases involving safeguarding, mental capacity assessments, hospital discharges, and continuing healthcare.

Work collaboratively with health professionals, housing, and voluntary sector partners to provide integrated and person-centred support.

Carry out safeguarding enquiries under Section 42 of the Care Act, ensuring timely interventions and risk management in line with statutory guidance.

Maintain accurate, up-to-date case notes and documentation on Liquidlogic (or equivalent system), ensuring compliance with council procedures and legal requirements.

Maintain Social Work England registration and engage in regular supervision, CPD, and team development activities to uphold professional standards.
